A good outage report describes what failed, when it failed, and how broadly you tested it. Name the service and action, include the exact error wording if it contains no private details, and give an approximate time with your time zone. A broad region may help; your street address will not.
Say whether you tried another browser, device, or network, and whether other users report the same symptom. Check the service's official status page and community reports before calling it widespread.
Remove email addresses, account IDs, order numbers, session links, tokens, payment information, and personal messages from screenshots and logs. Use a synthetic example if the error can be reproduced that way.
