An approval gate can improve routine triage, but it must not hide urgent cases in a pending queue. Define which decisions need a human reviewer—priority changes, customer-facing escalations, or closure recommendations—and which alerts should page the on-call owner immediately. Record the evidence the reviewer sees and the exact decision they make.
Set a response time for the gate and a fallback owner if the approver is unavailable. A rejected triage result should return to a named person with a reason, not silently disappear. Test an urgent incident and a low-impact request to confirm both routes. Keep the ticket’s original timestamps and any customer commitments visible throughout approval.
