A meeting-note draft can look polished while assigning the wrong decision or owner. Compare the sample with the recording, transcript, or contemporaneous notes where permitted. Check decisions, action owners, deadlines, and unresolved questions separately; these are not interchangeable.
If the record is ambiguous, ask the participants for confirmation rather than inventing certainty. Mark proposed actions as proposed until agreed. Correct the draft before broad distribution, and notify readers if an already-shared note contained a material error. Keep the corrected version linked to the original meeting and date so the team has one reliable reference.
